In a perverse twist to the “it came from a computer so it must be right” phrase, flawed accounting software in the UK postal system resulted in widespread underpayments from sub-postmasters (private contractors doing the work of a postmaster), who were then held accountable. Unfortunately, there was apparently no independent audit which might have exposed the flawed software which caused these people to be fined and in some cases jailed.
